CAGAA contracts with Delightsme to provide resident service coordination for GAV residents.

Delightsme specializes in providing aging services, service coordination, supportive services, and educational programming focused on empowering senior residents to live independently.

The Resident Service Coordination program provides individual assessment and Activities of Daily Living (ADL) for residents; organizes programs with topics of interest to residents; establishes links to community agencies and service providers; develops and maintains a resource directory; provides basic case management and referrals; and monitors ongoing provisions of services from outside agencies.

USC Community Health Involvement Project (CHIP)

This is a program provided by USC CHIP. The students from USC CHIP will arrive GAV with the proper medical equipment to help the residents get free blood pressure and blood glucose screenings on a monthly basis. They will also do a short presentation regarding diet and exercise while the residents are waiting to get their blood pressure and blood glucose results.

Dial-A-Ride

Dial-A-Ride is a shared ride service that our residents can make any trip from their home within the City of Monterey Park and trips to outside communities within 5 miles for medical appointments only. The Service Coordinator assisted the residents applied the D-A-R card and arranges Monthly Costco group trips.

Wheelchair Repair

The Repair Division Coordinators from Aging Care come to GAV to educate residents on safety, and to make an assessment of the residents' equipment, like wheelchairs-powered and manual, and power operated vehicles. Aging Care technicians arrange for quarterly visits to continue their assistance and reach out to residents who missed the last visit.

Educational Lectures

Service coordinators holds health and welfare related presentations for residents every month. These educational lectures promote intellectual stimulation, physical well-being, and social growth that enriches the lives of residents. Service coordinator will also translate lectures to Chinese as needed.

Hearing Aids

The Service Coordinator invites local hearing aids provider comes to GAV to educate residents on hearing health and make assessments of residents' hearing problems. The provider also arranges quarterly visits to maintain their services.

Mental Health Support Group

The Service Coordinator and USC interns conducted depression support group for residents who are suffering depression or anxiety. We are aiming to provide support and help group members learn how to regain control of their emotion so that they could cope with and overcoming problems.

Advanced Directive Support Group

An Advance Directive is a document that lets you make your medical decisions in advance, in case you lose the ability to make them for yourself. Although very important, it is not widely accepted within the Chinese community. Service coordinator and her interns conducted an Advance Directive support group to educate residents about Advance Directive and aims to help them complete this medical document.

Music Support Group

Music could help older adults deal with age-related problems such as general stress, depression, chronic pain, and even memory impairment. The Service Coordinator recruits an Occupational Therapist as our volunteer to conduct music support group and enhance the residents' physical and mental health.
